Looking for insight on localized skin reactions. My wife was clear for all of 2.5mg and the first 5mg dose, but starting with the second 5mg shot, she began developing red, itchy/firm bumps at the injection site exactly one day after the dose. We just finished the fourth dose of 5mg and the reaction happened again.

Has anyone else experienced this "delayed" onset only after moving up in dosage? Did it eventually resolve as your body acclimated, or did you find specific ways to mitigate the redness and itching?

We rotate the side of the stomach every week.

Not the red bump, but definitely itching the first six or so weeks of injections. I just took it as histamine reaction.

Delayed reactions are not mediated via histamine as by definition they are rapid, presumably if delayed it is a cell mediated immune reaction, just to be pedantic
Local injection site reactions are very common from any of the GLP's. Was noted in the studies, so most likely to the drug not contaminants or bac water. Apart from one person on this forum who gets them from all the GLPs, reta tirz and sema, cross reactions are rare so swapping to a different GLP will probably not have the same issue.
So long as the reactions are only local with no signs of rashes or other issues elsewhere, it just comes down to how itchy and annoying they are, they are very unlikely to go away. So you either put up with them or try antihistamines to see if it helps or swap to a different GLP.
No real harm in trying different bac water, but I doubt that is the issue. You could try a injection of just the bac water to see if she reacts to it to be sure.

I didn't have any reaction for my first 14 injections, going from 2.5mg up to 7.5mg. On my 15th shot, I got a delayed reaction (small, itchy, red lump) about 2 days later and it lasted for about 2 days. This week (my 16th shot), I got another delayed reaction 1 day after, but this time it's still here after 3 days.

I'm going to switch it up for my next shot and use my thigh to see if that makes a difference.

Not sure why my body waited 15 weeks to react to the shots, right as we're getting into beach weather here. Oh well 🙃.

Basically that is how you get allergic to something . You have to be exposed to it so your immune system can decide if it wants to react to it or not. Those sort of reactions usually take weeks to months , but usually not days or years. In theory grey peptides might be more likely to cause reactions with impurities acting as immunising adjuvants, but reactions are very common with the legit versions as well.
I inject everything else into my abdomen but mots-c into my thigh as I do get a reaction to it and it is not as bad or at least not as annoying in my leg.

Using official mounjaro I started with these annoyingly itchy red lumps to tried everything that was suggested but to no avail. Then changed to gray in 4 pins and no red lumps or itchiness at all and in for 64 pins in total. But also I stopped using the alcohol swaps in skin and just used soap and water but don’t really think that is the difference
The red lumps were quite distressing usually itchy on night times
