Hi All,

I'll try to give a background about myself, as well as some recent lab results and a discussion about my goals and experiences so far. I'll also include the peppy's that I'm currently running.

I'm a 37 year old male who has struggled with obesity my entire life. Looking back, we never really did have good food choices in my home as a kid. I never really learned proper eating habits and nutrition in my younger years. Luckily, at 14, I started playing football and began to have an obsession with weight lifting. I also wrestled, and transitioned into indoor track to throw the shot. I competed in a few amateur strong man competitions in HS as well. I graduated, 6'3 335 and ended up going to college, played football there as well. My college journey was cut short, mainly because of poor decision making and lack of guidance when I needed it most.
I continued training, competed in more strong man competitions and raw power lifting. This whole time though, I was considered obese. However, I was very strong. Bench press was 505 at age 21, Squatting and dead lifting well into the 700 range as well.

A neck injury in 2010 derailed my competition journey. I never sought treatment for this neck injury. I suspect it was a C7 disc issue, as I had constant deltoid and forearm numbness that transitioned into my hand. This was an uncomfortable few years, which lead into Opiate usage. Opiate usage ended up becoming a full blow an addiction over time. 2012-2014 was pretty horrendous. Poor decision making, prioritizing all the wrong things and nuking any meaningful friendships I had in my life. An accidental overdose prompted a response from the local police, first officer on scene was my old HS quarterback. A great man who I credit for saving my life. I was transported to a local hospital that I was formerly employed at. I was fully prepared to be discharged and go back to my shit head ways. One of my former coworkers contacted my family, and they came. At this point we didn't have much of a relationship, but they still showed up. This was not the first time my family had seen this type of event. I ended up moving in with my dad for a few months. I told him I could not be trusted with money. I slept on his floor, and I went through full on opiate withdrawal. Over the course of a few weeks, I was able to sleep. I started to eat better. I started walking, being outside, and enjoying being solely focused on myself. I remained out of work for about 4 months. January 17 2024 marked 10 years free from opiates for me. Luckily I never went down the rabbit hole of Methadone or suboxone. I did it on my own, somehow. Ended up rebuilding my friendships, one of which lead me to an employment opportunity as a rigger, traveling the US, Canada and Mexico.

When I was on the road, I ballooned up to 394 pounds, the heaviest I'd ever been. I was thicc, in all the wrong places. I ended up changing employers in 2017 and was on a ladder restriction due to my weight. I made a change, and by using conventional methods I was able to get down to 291. Over the last 5 years I slowly crept up to 321 pounds. A few months ago (Jan 25), I decided to try Triz, starting at the common trial dosages.

From that time, I've gone from 321 to 299.2. I have lab work (Belong to a telehealth men's clinic) from 10/1/24, and more recent labs from a few weeks ago. Results are attached to this post.

These are the compounds I'm currently using.

Tesamorelin 1MG 5x weekly
Tirzepatide 7.5 MG 1X weekly
Enclomiphine 12.5 MG 1X daily
Cialis/PT141/ Oxytocin troche 1/4 troche daily
Boron, 12 mg Daily (To combat my high SHGB) You'll see in the lab work.
D3/K2 combo, 10KIU daily

I've attached my most recent lab work to show the improvements over the last few months. The previous lab work is from 10/4/24 I think, the older results are in the column to the right. I also went for my first Dexa scan last week, but I'm not sure that will be of any use as it's my first one. I will do another in 6 months in share the results.

Positive trends noted in the lab work:

Lipid panel, huge improvements in every single marker.
Testosterone, Free and total, as a result of the Enclomiphene monotherapy.
LH/FSH, for the same reason as noted above.
A1C has improved.
Vitamin D has improved.
C Reactive protein has improved
Insulin has improved
Apolloprotein B has improved

Negative trends I noticed:
Reduction in IGF-1 (as a result of being on Enclomiphine) Hoping the Tesamorelin will help with this. I've also just had my first kid, so my sleep has been totally fucked.
My SHGB is SKY high. 12 MG of boron isn't even making a dent in this. If I can get this lower, my free testosterone will improve. However, nothing my mens clinic has offered has lowered my SHGB.

As I said, I attached the lab work if you want to view it. This is all paid via cash through the mens clinic I use. I mainly use them for my labs, all the pharmacies they use are extremely expensive. As an example, 48 MG of Tesamorelin was 1300 dollars.

As far as how I feel. It's the best I've felt in a very long time. Tirzepatide has totally changed my eating habits. No desire to eat out. No desire to snack late at night. I crave Meat and protein shakes and that's about it. My only complaint is that It took me a while to figure out the constipation. I use hemp fiber protein powder to meet my fiber goal. I take stool softener with my meals, and I take Magnesium Glycinate as well. It's saved my turd cutter, cause that thing was roached for a little bit.

Most recent DEXA scan shows that 266 would be an ideal weight for me as I currently carry 207 LBS of lean mass. So that's my goal. Trying to hit 250 grams of Protein daily. Meat always has been, and will remain the cornerstone of my diet.

idk if thus will help you and you've probably already read this somewhere, Boron is not the SHBG lowering answer, certainly not long term. I too had high SHBG levels at one point and tried Boron, 3mg up to 6mg daily, 100kg bw. The problem with Br, if it works at all, is that it has only been shown to have effect for up to 8 weeks in most studies. It also upset my stomach. I felt zilch from Br.

Lowering your BF, NOT intermittently fasting, and eating carbs are the best way to get into normal ranges. Obviously you are working on fatloss and hopefully you've seen some progress. Coming OFF keto was the biggest thing for me, I still fast at times, just don't do IF.

Its also been shown that high levels of animal protein can raise shbg. It's been a challenge but combining the right legumes and seeds/nuts to make a complete protein by adding up amino acids has helped me to still get enough protein to support/build muscle while cutting back animal proteins. I use a diet app that tracks AA but short hand is legumes and grains are missing methionine and cysteine, so look for seeds and nuts high in those and eat them with your meals.

I was not aware that Br is not a successful long term solution to lower Shbg. I do know that I've been on it for a year and have seen no improvement in my numbers.

I have read about animal protein pote tally causing an increase in SHBG. However I'd read information to the contrary as well. What I'm doing is currently not working. So I will follow your recommendations on adding legums. That will be hard as I typically avoid them altogether.

Intermittent fasting was my go to for a long time. When I started on triz two months ago I needed to make a change. I was in a caloric deficit and fasting made me feel terrible. My last meal is 1am (I work second shift) and I'm usually waking at 9 starting with some eggs or a protein shake.

I appreciate the advice. Will have to get up to speed on legumes !
 
On the legume front it took me a while to come around. I was a big time animal protein only kinda guy, 2lb a day at a be around 190-200lb for years and years. I've learned there are some pretty tasty lentils, the golden ones are good in curries, the black ones are awesome in taco bowls, green ones great in stews, humus is the shiz and since you're adding sesame seeds it's a complete protein.

One of the way if made it easier for myself is I replace half of my rice by volume with a lentil and seed. Through it all in my rice cooker and bam. Admittedly it's not as good as cooking separately but hey, goes well with some bison and hits my macros lol.

Here's some screenshots to help if you decide on pairings to pursue plant based protein sources. I use mynetdiary app to fill in amino acid profiles, no other app/website let me do that at that time.

Thank you. I just wanted you all to have a transparent view. To add clarity, I'm down from 321 to 299 in about two months. I started Triz at the end of Dec 2024.
Ah! That makes more sense! You're losing fast! My best advise (you didn't ask for lol) is stay at effective doses as long as you can. I stayed at 5mg the duration (-70lbs) and I didn't get on the scale at all. Just my 2¢
