I seem to get these "dehydration-like" headaches a day after I take a dose of peptides. Currently on a combination of 5MG Tirz and 0.5MG SEMA.

The headache feels like it's right at the base of my neck and the pain radiates through my neck muscles. It usually takes a combination of OTC pain relievers to nip it in the bud. But sometimes it feels like I could get an ocular migraine from them.

My current theory is that with the slowed gastric emptying that water absorption is also slowed and if I workout or do an activity that causes me to sweat, combined with weekly injections is to blame.

Seems like a dumb question, but is it reasonable to believe that hydration is also slowed along with slowed gastric emptying?

I haven’t really thought it through as far as you have, but from a quick search that seems to be the case. my solution is just more water and more electrolytes, more frequently

I think it’s possible. Do you tend to urinate frequently when hydrated? And do you include electrolytes in your routine? It might help to increase your hydration the day before and on the day of your dose, ideally with an electrolyte supplement. Pre-hydration is always more effective than trying to catch up when you're already facing potential dehydration.

Also, what are your hydration goals? I've read to aim for half your body weight in ounces. I'm around 200lbs so I aim for 100oz a day.

Well I have a diet coke habit. And it makes me urinate frequently which I assume is because it is a diuretic. I've been slowing down on the diet coke and sticking to water.

I casually drink water throughout the day and with meals. I don't really measure my water intake but that is a really good suggestion.

I have been buying Gatorade powder from Amazon. And when I start getting the headache is when I'll drink a couple glasses of it. Good suggestion to pre-load.

Not dumb at all @brentm!
Slowed gastric emptying certainly is part of the likely cause. It slows uptake of water as well as nutrients in our food. Further, gastroparesis makes it physically uncomfortable to drink large quantities of water - my tendency is to avoid it 😀

My reading surfaced a factoid that surprised me - we normally get 20% of our water intake from solid foods. Since starting GLP1s I eat less than half as much as I used to do. The average restaurant meal that I used to completely consume now easily provides enough to take home for a second complete meal, sometimes more!

The generally uncommented result of successful GLP1 therapy is that our water intake is immediately reduced by 10%.

Add to that the very real physical disincentive to drinking big volumes of water that slowed gastric emptying creates and…. Well it is a wonder there isn’t an epidemic of dizziness, fainting and other dehydration-related ills.

Looking back I have to consider that my favorite side effect while on Sema, constipation, was likely aggravated by insufficient water intake. 🤷🏻‍♂️

I bought a ton of liquid IV packets at Costco when they were on sale. Even when they're not on sale, they're a great price compared to most places. Super convenient and effective.

I hadn't considered that part of our hydration comes from food, but that makes total sense.

I too thought that the constipation factor was likely the result of dehydration.

I could swear that something about the initial sawtooth spike in Sema in my system just caused my hydration to do strange things given it felt as if all other factors were equal.

GLP-1s don’t just reduce food cravings, they also diminish thirst. This can lead to drinking less than usual, so it's important to make a conscious effort to stay hydrated. Due to weight loss, your body actually needs more fluids.

To help your body absorb water more effectively, you can take electrolytes.

I was getting very low blood pressure, dizziness, etc. when I first started tirz. For the last several months, I use one LMNT electrolyte packet every single morning - one packet in about 69 ounces of water. Every day no matter my activity level. I try to drink it all before any coffee. Then I drink another 74 ounces of water during the day without fail, and often more. At least 143 ounces of water every day. LMNT is more expensive but I hate the taste of the other electrolytes especially Liquid IV, etc. The GLPs definitely slow hydration - and the electrolytes help your body actually use the water to hydrate. TMI for sure but I used to drink water and just pee it out, I was constantly peeing, now with LMNT, I pee far less but far more when I do. It's amazing.

I couldn't resist memeing that.

And I just realized I misunderstood your statement. I think you mean LMNT is amazing. But I thought it was peeing with LMNT is amazing.

I got myself one of those large water measuring cups. Fill it up in the morning with my Costco electrolytes and get about 120oz in a day. It has helped me get to my goal hydration levels. And the Costco liquid IV electrolytes make it taste good also.

I have always drank a glass of water with electrolytes with my dose. And drank a ton shot day. I already consume around a gallon a day. But I was very lucky to be instructed from the get go to do that. I didn't 2 times in 7 months. Never again...my migraines trigger in my neck and this kicked them off
