Been on 15mg Tirz (mounjaro) for 6+ months. Want to make full switch to Reta ie end game of making no tirz and only on Reta. Started Friday and dropped straight to 7.5mg tirz then on Monday took 2mg Reta. Was going to stay with this mix for 4 weeks then switch to 5mg tirz with 4mg Reta and so on. Interested to read other members journey with dosing up Reta and down tirz and what your journey looked like timeline, dose transitions and if you landed successfully only on Reta.

Probably not as helpful as my doses were lower, but I didn't mess around with schedules or weaning, etc. One week I pinned Tirz and the next Reta. I looked up what dose might be equipotent (tirz to reta) and went for it. Reta has a little longer half-life (6-8 days vs Tirz 5 days) but stayed on Sunday morning pinning schedule. I did fine. This was back in August and I haven't looked back.

What dose of reta did you start on and what are you on now?

How did you estimate the "equivalent" dose for reta? I'm current taking 5mg tirz every Thursday (still my first vial) and planing to make the switch in the next two weeks.

I would very much like to know this as well.

There really is no exact formula to translate the dose of tirz and reta. Based on weight loss effects it would be something like 15mg tirz is about the same as 10 of reta. But there are side effects from reta that are less of an issue with tirz, mainly those relating to sympathetic nervous system activation like increased heart rate or insomnia, and skin sensory issues like allodynia. Tirz does not affect glucagon receptors but reta does and this is the main reason for the difference in side effects, plus tirz is a bit stronger on GIP than reta and GIP counteracts the nausea and vomiting from GLP-1, so reta can have more of those side effects as well.

So swapping to 10mg is a pretty risky move, that could leave you with a week or more of very unpleasant side effects, and there are stories on this forum of people having problems swapping to doses as low as 6mg.

So probably safer not to swap higher than 4mg of reta as a first dose and see what happens, but increasing a bit faster than every 4 weeks is not very risky, so long as you take into account the fact that any mild side effects at a given dose are likely to get worse as you increase doses and take that into account in deciding to increase doses or not or when to do it.
