I am down 20 lbs , working out and eating right. I am starting to feel very hungry at times. That first week was wild and I was hoping every week would feel that way. Wondering if I should titrate up?
Thats a really great loss! As long as you're still losing you should just stay there. Less side effects, etc 🙂I’m on 2mg. I also eat well and workout. Very active. It’s a little easier when my start was 385. I lost 55 lbs naturally before that.
After 4 weeks if it were me. I didn't feel the same at 4 as I did with 2, but still losing roughly 2lbs a week, and just popped 6mg Friday. The hunger suppression is big on this amount. My wife doesn't think so, but she's still losing. Everyone is different I guess. When would I want to titrate up?
I’m considering going to 4. Any reason I wouldn’t do a week at 3 before going to 4?After 4 weeks if it were me. Not at all. We actually did at 5 at 4, one at 5 and jumped to 6mg. A week on 5 felt like nothing, so let's go for 6 I figured. I'll know Friday if the 6 is rocking, that's scale day. Sure feels like it.I’m considering going to 4. Any reason I wouldn’t do a week at 3 before going to 4?

This is incredible thank you for sharing.@biobob this is Reta Phase 2 data. Rapid escalation produces greater weight loss. Slow escalation has better side effects.
People who started high (4 mg vs 2 mg starting dose) and escalated lost a greater percentage of weight. If weight loss is the #1 goal AND side effects are tolerable, then rapid escalation by 2 mg every month is the right thing to do per the study. If side effects are an issue, then follow slower escalation - go up by 1 mg per month. Your tolerance will dictate the rate of escalation.
Sides were an issue for me, so I escalated at 0.5 mg per month. Took me 8 months to get to 4 mg. I will lose less weight due to my slow escalation. But I got there without losing my mind and without quitting.

Since you're doing so well, I would wait until the weight loss tapers off. When would I want to titrate up?
I agree. Ride it until you flatline.ride it out for a while. if you are still losing then IMHO there is no reason to titrate up. you want to get the most out of the titration you can before you get to max dose, or have too severe side effects. if you stall for more than 3 weeks id consider increasing if you can manage hunger. when you increase id also say to give it 4 weeks before considering upping again. it takes about that long for levels to balance in the body to see if it is working as intended
I am sure a lot of it was water weight.Given the start weight at 385 lbs, you have a long way to go and will almost certainly need to get to full doses eventually, and I would argue 12mg is the correct dose for best weight loss , maintenance and more importantly long term health.
20 lbs in 4 weeks is fast, was weight stable before you started reta? You said you lost 55 lbs before that, if weight was stable even for a short while then 10 lbs out of that 20 could just be fluid rather than fat. I think weights over the next few weeks will most likely show slower more accurate weight loss, over 2kg a week is almost impossible for any length of time, unless you are very heavy , eating very little and active.
At the current rate there is no hurry to increase doses unless you are suffering with hunger, but I would suggest aiming long term for higher rather than lower doses. Most people on here are going to say use the lowest dose possible, I do not think that is correct for people with severe obesity, assuming side effects are not a problem.
I just placed my first order from grey. Here’s to hoping they come! 😂😂The simplest way to look at dose increases is, do you have side effects that are a problem? and are you hungry? / finding it easy or hard to stick to eating what you want to lose weight?
If no side effects and hungry increase dose
if side effects and not hungry - wait
if side effects and hungry - that is pretty annoying and there are no really good easy answers, I had that for a year on ozempic before I discovered grey peptides existed, maybe smaller more frequent doses.
Increasing doses by less than the standard amounts is always an option, it will be slower but will usually lead to less side effects. The recommended dose increases for reta and tirz are reasonable, the ones for ozempic are poorly designed and too rapid.
