Stalled on Tirzepatide titration after Zepbound 15mg – Should I jump to 10mg?

Looking for some perspective on titration schedules. I started at 350lbs and hit 196lbs on 15mg of Zepbound. Unfortunately, insurance stopped coverage, and after a failed stint on Wegovy, I rebounded to 230lbs.


Now, my wife and I are using Tirz60. We both started at 2.5mg. I’ve done four weeks at 2.5mg, two weeks at 5mg, and two weeks at 7.5mg. She has done the normal four 2.5mg and four of 5mg because of being new to it. I’m currently feeling zero effects/suppression, while my wife is responding well.


Given my previous history with the max dose (15mg), is it safe to jump to 10mg for my next shot tonight, or should I finish the 7.5mg cycle? Has anyone else who "re-started" after a high dose found they needed to move up faster?

IMO you can move up at the speed you’re comfortable assessing your sides.

I went from 5mg Tirz to a high dose of Reta quickly and painlessly with great effect.

It would not be surprising if your results now are less impressive than the first time around.

Congratulations on your great weight loss! Over 150lbs is quite an accomplishment and only gaining back ~30lbs is a testament to your dedication. Prior studies would suggest that the rebound would have been much higher.

Yes, prior exposure to a glp-1 medication often requires higher and higher doses when the medications are restarted. It's not uncommon for folks to need 12.5mg or even 15mg to start feeling the benefits.

Since you've already done two weeks at 7.5mg and are presumably tolerating any side effects, then if I were you I would definitely titrate up to 10mg now. Maybe do that a week or two that and then titrate up to 12.5mg. And then keep going. Don't stop and go beyond 15mg if you need to. I'm currently on 18mg and I would go even higher if I needed. I wouldn't mess around with this 4 week titration. That's definitely for someone's first rodeo. But let symptoms be your guide and remember that not feeling any hunger relief is a symptom that you need to go higher.
 
We all need to be mindful of where the recommended dosing and titration schedule originated. I’m sure BP knew that many (most?) patients in the studies could tolerate higher doses with no sides quickly. And that efficacy varied significantly between patients at different dosages. But defaulting to a four week titration schedule probably soothed the FDA…and delighted BP shareholders.

Honestly because of your prior exposure i dont think youll have any difficulty increasing your dosage to 10mg without any issues. You could do then do teo weeks at 10 and jump to 15.

When restarting a GLP1, youll find that the second time around getting the same appetite suppression may require higher dosage. I would try to get to 15 quickly and then reevaluate. Good luck!
 
The best guide by far about increasing doses is the side effects you did or did not get when you originally increased the dose at that level, if there were no issues last time, it is very unlikely there will be this time.
The other factor is how long you were off the drug, if weeks then you can probably restart close to the previous dose ,if several months , then a bit slower.
Most of the time if you have zero side effects increasing the dose will be fine, if you have mild side effects on the dose you are on they will probably get worse with dose increases.

Don't forget the 2nd factor in titration, frequency. Not to muddy up the discussion but I wasn't convinced 7 days was optimum. Not only that, I wanted to pin more than .5ml/15mg, so I pinned 4.5mg more 2 days later. Now, this isn't meant to be an optimum example, but still an example of possible pinning schedule. Honestly, I never felt splitting the dose exactly in half was the answer, but again, that's just me.
I think the answer has sort of already been answered, as go methodically, and go by feel.
