Ok. So i want experiences.
I have tried sema. Lost 46lbs. Have stalled. Not sure if i want to add cagri or just go to Tirz. Even reta.

I would love to all yout experiences with the 3. I've hears reta is the best, but tirz is the best at appetite suppression. I feel thats what I need since im a bored snacker. So how is reta better? Hell, I even snack when im full!

Not sure the route to take

Suggestions?
 
With Tirz, i lost 22kg (48 pounds) in just 12 weeks and had no side effects at all, and the food noise is completely gone. I'm steering clear of Reta for now since it raises heart rate (up to 6.7 bpm in studies) and is still in phase 3? trials, so it doesn't feel safe yet for me.....
 
Many different opinions. With that said, many people stack low dose Cagri for appetite suppression and Reta for active fat burning for weight loss. Cagri didn't agree with me so I stacked low dose Tirz and Reta. Even though I'm in maintenance, I still like the appetite suppression. I'm really looking forward to trying Eloralintide at a low dose for appetite suppression, but continue with Reta.
 
I can't see how reta does shit for me. I'm bombing 15mg of tirz. Ain't doing shit. I used to get punished for overeating. Not so much anymore. I haven't found any evidence that any of it is some magic shit. It's supposed to sort of train you. Well, I'm too stupid to be trained, I guess.

Do you mind me asking what your maintenance doses are and how you decided the dose and how often to dose for maintenance?
 
I've seen Sema has the highest appetite suppression score. RS did well on it until it stopped working. I added Cagri, bc amylin not glp, and it had a noticeable impact. Moved RS to Tirz the beginning of the year. They are doing well on T.

Good luck. Took me some time to settle on a decision how to best move forward.
 
I started at 336 in late 2022 and lost a little down to 318 on my own. Then lost 106#’s on Sema from mid 2023 to early 2025 and stalled for a couple months at 211. Tried stacking 3mg Sema with Cagri, it gave me horrible constipation. So I gave up on Cagri. I lost over 33% of weight on Sema which is comparable to weight loss surgery. And a total of 38% at peak weight loss. So a great victory. Tried moving over to Tirz then Reta. Reached max dose on both at separate times with no weight loss. Finally gave up and moved back to 1mg Sema to mostly maintain and slowly gained for a few months. Reached 224 end of 2025. I'm now refocused and trying a Sema/ Reta Stack currently at 218 and hoping to get to 190. Stacking 1mg Sema on Monday and just did 4mg Reta today ( Thursdays) as increased stack from 2mg for four weeks. I need the hunger suppression that Sema gives me. That Tirz or Reta Do not. I basically felt like Tirz or Reta did nothing by themselves. I may try Tesofension with Sema if this stack does not work. But have not seen many encouraging reports out these. So far Sema works best for my hunger, food noise, evening snacking. I will titrate up on Reta and will add a little more Sema if needed for now. I may try Tesofensine with Sema if this does not work. I'm not hoping to get to a super low weight as I'm 5’6”. The experiment continues.

Reta better at sparing muscle

No, it's not:

Lean mass loss comprised the remaining 35.4% of the total weight loss, a proportion that is “similar to other obesity treatments,” the authors write in The Lancet Diabetes & Epidemiology.

Despite all the reta bros, some studies show that tirz has a lower percentage of muscle loss than reta or sema.

Still a work in progress. I maxxed out at 12mg Reta and 5mg Tirz. I went off Tirz for a bit and felt hungry. I tried Cagri and got headaches 24/7 (and I almost never get headaches). I then went back to 4mg Tirz and I am currently on 8mg Reta. That has been stable for me for the last several months. Could I go down on Reta? Probably. But I'm happy with the stable weight and with the appetite suppression. I plan to stay here for the near future. Dosing is 1 time per week, but not on the same day.

I can't speak to effectiveness of reta, or sema but after only 6weeks on Tirz the difference is staggering. I have fought weight and obesity all my life. This is the first time in years I don't feel hunger and food noise constantly pulling at me. The results this far is unbelievable. I'm already seeing very noticeable physical results.
 
I run Reta 4 mg every 84 hours. The last 12ish hours get tough with hunger for me. Definitely less hunger suppression than on Tirz, BUT, high dose Tirz was hell for me. I struggled to function for about 72 hours after that injection. I felt like I got ran over by a truck. Plus hit an extended plateau (10+ weeks) and I couldn't see continuing on.

The scale isn't moving much with the transition to Reta (went from 12.5 mg Tirz to 8 mg Reta in 8 weeks, titrating Reta up 2mg every two weeks), but I've noticeably lost fat around abdomen, hips, neck. I personally don't care what the scale says if my clothes fit better.
 
Equally for me.
But when i stack just 0.1mg of Sema with my 5mg of Triz, the apetite supression was perfect.
Depending on how Reta goes for the next month or so, I may consider adding in a little bit of Tirz, like 2.5 mg, or perhaps a low dose of Sema as well, like 0.1 or 0.2.

I have noticed FAR fewer GI sides on Reta and that has been a real blessing for me! The daily famotidine and prophylactic Gas-X on 12.5 mg of Tirz just to get through the day was hell.

That's where I am to when I started on Tirz I didn't even have a scale in the house (to depressing to even step on one). Six weeks latter I'm wearing jeans I had packed a way two years ago that I could not fit in to and I'm wearing a coat that I previously couldn't zip up. I'm so happy with the progress that I actually bought a new scale for my house for the first time in probably a decade.

February 2025 I weighed in at 429.5 at a doctor's visit and he had a sit down with me to discuss everything I already knew but was too lazy, stubborn, weak to do anything about. Dieting was always a rollercoaster on the scales and I inevitably always gained more back that I started with. From February to early July I was counting calories and choosing to take the stairs instead of the elevator, parking in the back of lot, etc to try and reduce the burden. I was able to lose about 20ish pounds in that 20 weeks, doctor preached slow and steady. Well, I wasn't satisfied, went down the glp1 rabbit hole and ended up here.

I joined in July, created my 'about me' in August, and started Reta in September. I started at 2mg and am now up to 8mg and doing pretty good. Yesterday, my weight was 357.7 so that's 50ish pounds in 5 months with very little exercise added in and admittedly bad tracking of total calories. I am more focused on consuming proteins since I just don't feel like eating half the time or we go out to eat and I can only eat a third of the meal.

Interesting, my scale thinks I have added 7.2lb of muscle mass. Which obviously isn't going to be accurate.
 
I previously used compounded semaglutide and had some success, but the side effects were enough of an issue that I did not want to continue taking it. I recently started taking reta. I noticed an immediate reduction in appetite on week one with .5 mg. It started to trail off by day 5 or 6. I slowly ramped up to 2 mg, and the appetite suppression is lasting all week, but some days it is challenging to eat enough to get protein goals with squeezing a protein shake in somewhere.

I was a boredom snacker, a watch a movie before bed with some ice cream snacker, a run upstairs and grab a coke and some chips snacker in the middle of work snacker, and the idea of snacking right now doesn't even sound appealing.

When I started the reta, I stopped buying those things because I just did want the temptation to be there, so that helps some too, but now eating has become more about trying to eat enough to get a good amount of protein and fat and less about grazing all day. I may have had similar results had I stuck with the Sema, but I had noticeable negative effects on sema and only one small side effect so far on Reta that is more of an inconvenience than a problem.
 
I thought about food all the time. If I started a bag of chips I couldn't stop. Taking Reta I have zero interest in food. I struggle to want to make dinner for the hubs. Nothing sounds good. Now if I start to eat something really tasty I could def eat it but I just don't keep it in the house. I'm just focusing on getting my protein in. And it completely turned me off of any alcohol, which I miss a little.🤣

LOL, in my case I never went to a Dr to get the lecture I have a family member who is an RN that has been berating me for years that I needed to slim down. Even on the occasions were I actually tried I made 0 progress. I had a friend who got put on Ozempic for diabetic treatment as expected shr lost a ton of weight. Without insurance that would pick up the tab that was a lost cause. Then low and behold South Park did a special "The End of Obesity" and I started doing some online snooping. A few months later and weights way down spirits are up for the first time in many years I have started to enjoy life again!

As a recovering alcoholic 3+ years into sobriety, tirz totally killed what residual cravings for booze that I still had. Dr had me on max dose Naltrexone for a year - tirz did a better job and enabled me to shed 100 lbs as well. Win win win. 🥇
 
I'm a super slow responder in general. With following diet and working out, it still took me 18 months to reach 150 pounds from 190-195. I've been plateaued at 150 for a year now.

But I started on sema and stalled so I changed to Tirz and lost the rest of it. When I was sick of the plateau, I tried Reta but gives me diarrhea once I get to 4mg. I tried dosing 2mg every few days and still my body couldn't handle it. I'm now on Tirz 8 mg a week as maintenace as my body seems happy being 150ish. I'm more interested in gaining muscle vs the number on the scale anyway. I was borderline prediabetic so that's why I stay on Tirz.

To those that found tirz killed their appetite for alcohol, what is your dose? I have not found this for me (wine drinker) aside from the first month or so. I’m now at 5mg every 6 days.

From 10mg on up, @ 15mg now. Strangely, Wine is the only thing that doesn't make me nauseous. I used to be a big tanquerea gin drinker. I don't know much about the crave. I never did have much of a crave. Just knowing how sick it'll make me is enough.

Thanks! I don’t typically do a lot of hard alcohol but every two months or so enjoy a vodka martini. I guess it all comes down to awareness and moderation for me.

In the background of the diet and the medicine/peptides the choice to take the stairs instead of the elevator, parking in the back of lot, etc Is a AMAZING thing to keep doing, I also added cutting all surge drinks etc. Keep up the good work you will get there.
