Re the shoulder pain, do you maybe have "impingement?" I put off shoulder pain for a few years thinking it would be a rotary cuff thing, which would require surgery. I was to the point where I could not reach behind to put on a belt, nor reach much above my head, it was so bad, before I finally re-complained to my doctor for the 2nd time in 3 years.
A few weeks of daily light strength training targeting the shoulders, and some gentle stretching, and I was about 70+% better. Still working on it.
