I think the quote is a poor way of expressing an important underlying concept. Nick Norwitz's recent critiques of CICO are similar. Valid concepts, but phrased in a way as to intentionally trigger midwits who can't properly grasp what is actually being said.

People don't become obese because they "accidentally" eat too much. As they're gaining weight they're very aware that they're eating too much, but there's an underlying compulsion that is going to be very difficult to resist long-term. It's as if something in their metabolism changes in a way that causes their built-in satiety mechanism to lose its calibration and drives them to overeat.

I'm not sure if this is what's happening, but I'll throw a potential model out there:
Let's define food (carbs, protein, and fat) as fuel and ATP as energy.
Your mitochondria convert fuel into energy to meet energy demands and in a metabolically healthy person the process just works. In someone who becomes metabolically deranged, the process is impaired such that your cells experience an energy shortage. Your brain assumes this energy shortage must be due to a fuel shortage (even though there's plenty of fuel) and triggers a slightly stronger hunger signal than makes sense for your current fuel levels. Over time this imbalance ultimately becomes obesity and/or diabetes.

The mainstream explanation (often dubbed CICO) that people simply forgot to eat less and are too lazy to exercise sounds good on the surface, but obviously doesn't fit with the experience of most obese people.

May the Lord have mercy on our lab values. Mine are relatively perfect now, but I was still mildly obese when they remarkably improved on GLPs. I did not check fasting insulin though, which would have been interesting:

Awhile ago I was told by a neighbor that she and her whole family had the "obesity gene" and alas there was nothing they could do.
It got me wondering, so I started to look around and watch people and what they consumed.
At the supermarket I looked at everyone's carts, and how healthy those people seemed.
I noticed direct correlations between what they bought and how they looked.
Obese people's carts were full, and they were full of pre-prepared foods, and sugary drinks.
On he opposite side of the spectrum; fit athletic people were buying raw foods and in small quantity..No pre-made foods or drinks.
The neighbors had a fridge in their garage, so the kids could grab all the sodas they wanted at any time. They also had see through trash bags, and on trash day there were 2x as many bags as our house, and they were full of previously frozen and pre-made food boxes.
Now this wasn't a scientific study, but to me the results were clear. What ever they are putting in those ready made meals is causing obesity, and so are sugary drinks, but so is the attitude that eating and drinking whatever and as much as you want is healthy.
