Reta ”build up”?

Hi! New to reta and been reading so much about it im starting to get confused now instead.

Some say you should start at a low dose then titriate up slowly. Other say start at 2mg and up it 1mg/week.

I chose to start low and titriate up, but now in also hearing people say you should stay on a low dose for four weeks because thats the estimated time it takes for the body to get used to reta and start getting the full effect.

What are your thoughts? Are there any real facts about this or just peoples own experiences?
 
2mg/4weeks-5mg/4weeks-6mg/4weeks. Next is supposed to be 9mg, then 12 if needed. I stayed on 6mg for 6 or 7 weeks till my weight stalled and I lost all the “Reta” feeling. Went to 8mg and am still here. I was due for #5 Friday, but I’m away and will get it Monday morning. I’ll try to stay on 8 until it seems to stop. Then maybe I’ll go to 9 or 10. I’ve lost 48lbs since 1/9/26. Good luck.
 
Look up glp plotter it will show you estimated blood levels of the drug over time on a graph so it is easier to understand.

The reason you are supposed to increase doses slowly is to avoid getting hit by sudden severe side effects, around 1% will need hospital treatment from starting or increasing doses of GLPs due to dehydration from nausea and vomiting. This is what you want to avoid. Not the only side effect, but the most common serious one.

After a dose it take about 24 hours for it to be absorbed and reach peak levels in your blood , this is typically when the worst side effects will happen. Then it takes 6 days to drop to half that level, this is relevant because if you do get severe side effects or even less bad ones it can take a week for them to settle down. Each dose you have after that builds up in your blood stream and it takes about 4 weeks of weekly doses to get to the maximum level, so you can get worse side effects several weeks after a dose increase. This is why you are meant to wait 4 weeks to increase doses, as that is how long it takes to get to the new steady state blood level from the previous dose increase.

The standard recipe worked out by the company that made the drug is designed to have a fairly low risk of severe sudden side effects, which is start at 2mg and increase by 2mg every 4 weeks after that until you get to 12mg. If you are not severely overweight you may not need doses that high, if you have a BMI of 35+ you probably will.

For reta the common side effects are nausea and vomiting and constipation and diarrhoea, also skin sensory symptoms are fairly common and some people notice a increase in heart rate.

The odds are you will get some side effects at some stage, but there is a lot of individual variation in where this happens, one person might start to get nausea at 10mg and another might vomit for a week at 4mg, and about half will not get any nausea. The useful bit of advice I can give is adjust the dose increases based on the effects on you, If you get good appetite suppression at lower doses you may not need to keep increasing doses, if you get mild nausea at one dose the odds are it will get worse when you increase the dose so that might be a good time to slow down on increasing the doses.

Start at 2mg and titrate up. Stay on the lowest effective dose that works. If you stall, then increase. I stalled for 2 weeks at 9mg(total 6 weeks)and increase to 12mg/week. I did that for 2 weeks and that worked. I've currently hit my goal and am maintaining at 2mg/week atm.
 
I just simplified it a bit , as I think in general those bigger jumps from 6 to 9 and 9 to 12 are more likely to cause issues than going 6,8,10,12, but yes you are definitely correct, yours is the correct protocol.
 
I started at 0.25mg and increased it weekly by 0.25mg until my current, 4.0mg. It took a long time and I didn't have great results. I also added 0.50mg of Cagri to quelch the food noise. This combo works for me. Everyone's mileage may differ 😉

Lots of prudent people start at 1mg or less and titrate slowly to avoid shocking their body and lifestyle with the drama of nasty side effects, diarrhea, constipation or feeling ill.
Plenty of people begin losing at lower doses as well. Our bodies react differently.
 
After reaching goal, did your appetite return drastically when dosing down from 12mg to 2mg?
The whole goal is to improve habits and eating better. Thankfully on reta, you don't totally lose appetite and I was able to eat cleaner. I titrated down to 2mg slowly, not just in a week. I was going to totally get off of it but the other benefits(high bp) are worth the maintenance dosing. I may even space every other week instead of weekly.

There are two main approaches, as far as I can tell: One is to max out the dose/results as fast as possible. Speed-run the weight loss chapter and then move on to maintenance. The "biggest" results -- losing up to 30% of your starting weight -- come from taking the max dose for as long as it takes. The other is to seek and maintain the lowest effective dose for your goals. One study found that taking 1mg/week for a year resulted in a loss of 9% of your starting weight.

I'm kinda in the middle. I started on tirz in December and like it a lot. When I stalled in March at 1.66mg/week, I added 2mg reta (titrating up for a month) with the thought that I'd speed up weight loss and then go back to tirz for maintenance. The weight loss is working so far. I'll have to see what happens in maintenance.

As far as "real facts": There are research protocols, which mostly take the high-dose approach. Then there's individual experimentation, where people figure out if they respond well to low doses, or need to move up to get the desired effects. I don't think we know yet in advance who will respond to low doses and who won't, so it's good to stay open and let your body decide what the best dose is for you.

Consider yourself a fascinating science experiment. Begin at .5mg and see if you can tolerate side effects. This is especially important if this is your first GLP1. Titrate up slowly, observing yourself carefully. Some people are happy staying at 2mg or even 1mg long-term, while others don't feel any changes on such low doses. It's not a contest! It's just information. Only your body can inform you about that.

Start low and go as slowly or quickly as you like, using your personal criteria. Some will push through ugly sides to drop weight as fast as they can. Others will avoid, say, vomiting by taking it slower. It's all up to you. Tune in to yourself and pay less attention to what others do. HTH
