I have a 2 month old Tirzep vial that was from a telehealth compounder. It’s in good shape. It has not been used. I would like to add BAC to dilute it down a ML. Is there anything dangerous about doing this. I understand from the faithful it’s already 2 months old. I am not concerned about that. Just want to know if I can dilute it down when ready to use.

Although it seems to be a non issue, I am not qualified to advise, there may be some reason I am unaware of.

I would ask, Tirz can be dosed pretty small... Why wouldn't you just adjust dosage as opposed to diluting?

I may have wrote that wrong. Basically I’d like to pin a one for one mg. The current bottle is at a 2 to one. My thought was adding a little more BAC could give it stability. Or am I just being stupid. I’ll go with the later.

The vial is currently this.

Youre not stupid but im hella confused.

Are you saying you have a 10mg concentration in 1ml of bac?

And you want to add an extra ml? What does the vial say?

Here it is. Sorry for the confusion.
A 20mg concentration is fine. More concerning is how long you will use that 60mg vial for if you are only shooting 2mg at a time.

I wouldnt dilute it. Only because I dont understand why you want to. Anytime you screw with a vial you chance contaminating it.
Thats what I was thinking. In my world of being a newb I was thinking a touch of new BAC could have given it some stability. But you’re right. Less is more with these precious gems.

Just keep it cold, only use clean needles in for the vial and 1x for your body. Hold it up to the light to look for shit growing in it whenever you are gonna use it. You'll be just fine. Congrats on getting a big ass bottle that will last you approx 1 million years. 😅

How many of us started with the compounders. A friend who didn’t know any better referred me to this compounder. I know a few people that think working a single vial for up to six months is fine if cared for properly. But honestly if I threw it all out that would be fine too. Just hate throwing it out if it can still be safely used. Thanks to everyone for your help and knowledge.

What the heck is that mix? I'd personally trash it. How many do you have?

I get that you are trying to make it easier to calculate the units. But you could calculate it once and write if down - no 😛? Or does your dose change often?

I would just recalculate for a dose and write it down. Put a little sticker on the vial.

I am trying to a few extra months out of the vial. I thought maybe adding some fresh BAC water it could help preserve the pep. The syringe is an awesome idea. Thank you. Always learning.

Get them here Bill, and great price for Easy Touch. I use them and BD.

Just use it!!! I loved BPI and I also spent huge amounts on compounded tirz at one time. Its like the price for entry lol

Your picture of the U-100 Syringe (a insulin syringe) is correct for 20mg of Tirz in 1ml of bac solution. To inject 2mg of Tirz, use 0.1ml (or 10 units on an insulin syringe) of that solution.

To answer your question, yes you can add 1 ml bac. You would then inject 20 units (0.2ml) of your recon. Adding fresh bac (with benzyl alcohol) to your vial will boost it resistance to new bacteria. At 2mg, you have 10 doses, a 10 week supply. That will be a total shelf life of over 4 months. Ouch.

Your goal of a one to one ratio is fine but is not a common medical target.
Manufacturers normally dictate how much bac, nurses need to use.
Lilly use 50units of bac per Tirz injection. Vaccines are often in 50 units of bac. Are more diluted. The reason: you 'may' get a skin rash when using only 10 units of bac for that 2mg Tirz dose (quite concentrated). But many folks use 10 or 20units, and are fine. I always inject doses suspended in 40-50 units of bac to avoid swelling.

Where was the vial stored? Room temperature? My compounder said max 3 weeks shelf life at room temperature. Refrigerated? then 60 days, from a compounder (using a sterile environment) seems fine, IMO. The expiry dates on mine showed one year away.

I use 60 day old peps often. Up to 90 days sometimes, in a not very sterile environment.
You could filter it into a new sterile vial to get 4 months. I have read some folks go up to 6 months. It all about sterility. And courage.

My children didn’t get treated as special as my Pepitides. They have been given exceptional care. I’m keeping a close eye on it. It’s no big deal if it goes on the trash. Just thought it be nice to squeeze a few more mg’s before it gets tossed.

What gauge are you using 30? Or lower.

I'm using 31g. Couple things about them. They'll draw the liquid slower from the vial, so it takes a extra minute to fill it. You only want to push it in the stopper once. If you're having problems filling the syringe and put it back in again, trash it and get another. It kills the needle and dulls it. Make sure to put the solution back in the vial. You can barely feel them.
