I've seen some conflicting thoughts as I've searched. Im using the max dose of tirz and researching what to add to keep the weight loss going.

Does it even make sense to add sema with the 15mg tirz? If so, what dosage?

I don't think Reta is an option for me because of risk to kidneys & health history. Cagri makes me nervous with ph balance and potential side effects.

What are your thoughts?

Yes, sema would be easier than reta, at least with dealing with known/unknown health effects. You would still need to be somewhat careful, of course, since any GLP can lead to dehydration, affecting the kidneys.

As you may know, some people who are wary of reta for elevated heart rate will use ghetto reta (tirz with survo or maz). So like tirz with a splash of survo. Anecdotally, ghetto reta has less elevation of heart rate.

Any unknown effects on the kidneys could be dose dependent. But Gemini advises there are known kidney risks with survo/maz/reta, like maybe the ones you are alluding to:

Glucagon agonists (like Survodutide) can increase "insensible water loss" via thermogenesis and potential diuretic effects. If you're stacking these without increasing fluid and electrolyte intake, the risk to the kidneys increases.

Testing blood pressure would be important, but it usually goes down with reta, as heart rate goes up. You could use GoodLabs to get things like Cystatin C tested frequently at Labcorp/Quest. But no need (or less need) if sema works out.

I am currently stacking tirz and sema. As far as what dose, the advice I received when starting was to start low and titrate up slowly. I've followed that advice and have been fine, despite AI cautioning me against stacking 🤣😂🤣😂.

Current doses are 17mg tirz and 1mg sema. I like the feelz of sema which affects me decidely different than tirz. It's a nice mid-week bump.

Totally agree about cagri. I have a kit of it, but probably won't ever pin. I'm probably going to donate to another researcher.

I would keep the cagri if I were you. I like it so far.

And there's more advice-mongering:

If you lose too much muscle, your metabolism drops, causing a plateau. Prioritizing protein and strength training often "breaks" the stall better than more medication.

Stacking these two is popular. Add low and slow a few days after your Tirz. I have been doing it for @2 years. I also stack a bit of Cagri once a week with it. Lost most of the weight on Tirz, then added Sema for more hunger supression. Then Cagri for a bit more and to lose the last 10 lbs.

Brimming with reta-free success! 🙂

Glad to hear it since I mostly have tirz hoarded away. I also like reta and really everything.

Instead of just getting more reta, I have been thinking of getting some sema since I don't have any more, and I will eventually run out of cagri.

Sema seems ideal for decade-long hoarding since it seems to be more of a roach than even tirz, with sema having a simpler structure than tirz or reta. Cagri would likely have a little more degradation than the GLPs.

Just as an alternative, why not higher doses of tirz? Once you get to max doses or start adding GLP's you are off the path of established research, but if I had to guess maybe higher doses of tirz would be simpler and maybe have less side effects.
You mention possible effects on kidneys. I do not know if this means minor early changes or actual renal function impairment, but if there is renal failure protein intake is an issue, but definitely a case for expert advice. Tirzepatide has definitely been shown to reduce progression of renal failure. I assume you have checked related issues like lipids sugars blood pressure etc. I think if you add other agents or increase tirzepatide above 15mg rechecking renal function after a while is likely a good idea.

I will no longer be stacking any GLp-1's because a peptide bro quack doctor with a beard and a baseball cap yelled on YouTube that stacking reta with tirz will permanently damage your metabolism. I don't actually know if he yelled all that, because the chances that I will physically listen to a peptide bro with a beard yell into a microphone on YouTube are less than zero, but I paused it and went and read the transcript, and now I'm spooked even though I know perfectly well that dude's a quack. You know what, that's enough internet for the day.

the other you tube doc, dr Jones says the opposite?

No need to stack GLPs when you can stack the GH peptides lol.

I add a smidge of in the ravenous days leading up to my period, keeps the crazy munching away.

And Sema is less expensive for the long haul

I stack Sema and Tirz but unlike most my main is Sema at the moment. I do a dose of Tirz midweek. It's a budget thing for me plus the Sema has worked well so far. The tirz does some good for me inflammation wise. When I finally get thru the sema I bought in the beginning I'll probably transition.

It's a test of will to keep from buying all the peptides. I want to try Reta at some point but I'm making myself use what I have since I have over a years worth in the freezer right now.

Even reta is getting so cheap now, so I don't know about the cost factor. Sema seemed cheaper last summer when reta and tirz were more expensive for smaller-mg vials.

I get tirz for 30 cents per mg (a year's supply is a T30 kit for $90 in faster-than-usual GB). On 6 mg now of tirz (before stacking), so that's $1.80 per week for tirz or $93.60 per year.

For reta, R20 now in a traditional, slow-ass GB for $95 (48 cents per mg) plus $15 shipping. R40 for $155 (39 cents per mg).

I guess I'm in a place of money already spent vs spending more.

It's okay to hoard your sema for a rainy decade, lol. But I admire your restraint. A few people I know have about 30 years of GLPs in their freezer, tirz and/or reta. Essentially a lifetime supply.

My concern is all of this cheap China reta gets expensive one day. Competing paranoias, with no right answer, since the prices may even go down further (as always, so far). I do know there is no need for me to go overboard since I can maintain at lower doses, though not as low as you maybe. And there will be more/better GLPs, lowering the cost of existing ones.

I stack them too. Sema is basically GLP1 with some mutations to prevent quick cleaving. No other GLP1RA is better than it at mimicking GLP1. Has the highest Ki for GLP1R. Its about 10 times more potent than either Tirz or Reta.

Tirz is based on GIP and has great affinity there. So biochemically they complement each other … or at least thats what i tell myself!

Either way, i lost weight, it worked great and broke through plateaus.

Now planning my maintenance.

I'm tempted to switch to the tirz completely every week. It is a test of restraint.

Part of it is also being an old lady and wanting to lose slowly.

Im going to say yes. I have not tried Sema/Tirz but I’m stacking 1mg Sema with 8mg Reta and back to losing weight after a long stall and 12 pound weight gain. I’m hoping to lose 10.2 more pounds for a total of 136#’s over 36 months. Hit my lowest weight 210.2 pounds in almost 20 years this week.
