I just pinned my 5th dose of tirz (2.5 mg) on Friday, and I've been feeling awful all weekend. I could barely choke down a protein shake yesterday and vomited today despite not eating anything. My fiance has been begging me to eat something, but the thought of food genuinely makes me feel nauseous.

I'm not sure if this is a delayed side effect or something, but I am not having a Good Time (and no, before anyone asks, there is a 0% chance that I am pregnant lol). Has anyone dealt with extreme nausea on such a low dose of tirz before? Or does anyone have any advice before I pass out? 😅

Your body is definitely still adapting as the drug builds up in your system over time. I feel like its your body telling you to skip a dose and try to get back to some sense of normal before beginning again.

I agree with the other poster. I lost my appetite after like my 4th month on Reta at 3mg and had to stop for two weeks and drop to 1mg probably going to further drop to .5. Eating is a must. Or you get the hair loss and that’s a no for me. I’m not mad about it, just means I’m a cheap date. Lol

First of all make sure you drink enough fluids so you do not get dehydrated. It might be worth trying to get some anti nausea medications.
It will settle down , drug side effects are typically worst 24 hours or so after a dose or dose increase and in theory it should take 6 days to have blood levels drop to where they were just before your last dose, so very unlikely to persist past then and most likely 3-4 days.
What is important is do not have another dose until the nausea and vomiting are gone, so delay the next dose if needed and then use a lower dose, like 1 or 1.5mg. And after that increase very slowly maybe only 0.5mg every 4 weeks, assuming nausea is not a problem.
In theory there is no really good reason to get this after the 5th dose at the same dose, drug levels should have been exactly as high after the 4th dose, so maybe double check if possible for a dose error, but glp effects and side effects do not always make perfect sense either. And it is still possible to get food poisoning or gastro on GLP drugs , so that is always a possibility.

I remember reading someone mentioned if you don't have/can't get Zofran something like otc less drowsy Dramamine (Meclizine) can help. Walmart has their own brand for a lower price too

I swear to God, if I could, I would have gleefully seized your problem, welcoming it with both hands. I wouldn't even care if folks thought I was pregnant.
A month ago, I would have agreed with you. But feeling too nauseated to eat also depletes all your energy and makes it really tough to do anything but lay around like a sickly Victorian child.

I haven't had side effects like this until the most recent dose, and I haven't increased the dose at all. I think I can also rule out food poisoning since I haven't really been eating much to begin with (usually under 1,000 calories/day).

My 1 month mark was awfull

insane GI
was on the bathroom every 20 min for days
stomac ache, food was a no go for 2 weeks total
i skiped 2 weeks of dose till i could drink water and started eating again
it was rought ...
but now i titrated back slowly and tried pin 3 time weekly rather than the huge spike of 1 day a week pin
for me the micro dose 3 pin a week is a game changer
Glp are trial and error
but they are worth the hassle to find the sweet spot

I tried to find a better description using google so forgive the copy and paste, but it is common that people have these issues when starting both from studies and I have known many that dealt with this.

It is worth considering that you may be having the lasting effects of a previous meal.

I would definitely be sure and hydrate.

Here is a description from a google search regarding slowed digestion and how it is the most impactful when first starting GLP1s:

START GOOGLE
GLP-1 receptor agonists (such as Wegovy and Ozempic) significantly slow gastric emptying, causing food to remain in the stomach for a longer duration, sometimes up to 48 hours in cases of significantly slowed motility, compared to the normal emptying time of about 8 hours. This sustained, <24-hour retention is a key mechanism for managing appetite, with studies showing 37% of a solid meal still in the stomach 4 hours post-ingestion. [1, 2, 3, 4]
Key Findings on Digestive Changes:
  • Initial vs. Long-term: The slowing effect is most profound in the first month of treatment, with significant adaptation (tachyphylaxis) occurring over 4–16 weeks as the body adjusts, although some delayed emptying persists.
  • Increased Retention Time: Studies found that at 4 hours after eating, about 37% of a meal remains in the stomach of those taking semaglutide, compared to almost zero in those not taking it.
  • Stomach Paralysis Risk: While common, some users may develop extreme slowing, known as gastroparesis (or "stomach paralysis"), which can cause lingering food to harden and create complications, affecting roughly 1 in 20 new users.
  • Common Symptoms: Due to the slowed digestion, users often experience early satiety, nausea, vomiting, bloating, and constipation, especially during dosage increases.
  • Surgery Warning: Because food remains in the stomach for over 24 hours, healthcare providers may recommend stopping GLP-1 therapy before surgeries to reduce the risk of anesthesia complications

THIS IS WHAT GOOGLE recommends:

To manage digestive side effects (like nausea, bloating, or constipation) caused by GLP-1s slowing stomach emptying, eat smaller, frequent meals, prioritize low-fat, bland foods, and stay highly hydrated. Key strategies include avoiding high-fat, greasy foods; staying upright after eating; and, if necessary, speaking with a doctor about adjusting the medication dose. [1, 2, 3, 4, 5]
Here is a comprehensive guide on managing GLP-1 induced delayed gastric emptying:

Dietary Adjustments
  • Smaller, Frequent Meals: Instead of three large meals, eat 5-6 small, nutrient-dense meals throughout the day to avoid overworking the digestive system.
  • Follow the "Bland" Rule: Consume easy-to-digest foods like bananas, rice, applesauce, and toast (the BRAT diet) to manage nausea.
  • Avoid Trigger Foods: Reduce or avoid high-fat, fried, greasy, and heavily spiced foods, as these stay in the stomach longer and trigger reflux and vomiting.
  • Increase Fiber Carefully: To manage constipation caused by slow transit, gradually increase fiber intake. Good options include fruits, vegetables, and whole grains.
  • Separate Liquids and Solids: Avoid drinking while eating. Drink fluids between meals to avoid overloading the stomach. [1, 2, 3, 4, 5, 6]

Lifestyle and Timing
  • Stay Upright: Do not lie down for at least 30 to 60 minutes after eating to prevent acid reflux.
  • Hydration is Critical: Sip water constantly throughout the day to avoid constipation and dehydration.
  • Slow Down: Eat slowly and stop eating immediately when you feel full to avoid overeating and severe nausea.
  • Move After Eating: Light physical activity, such as walking, can help boost digestion. [1, 2, 3, 4, 5, 6, 7]

Addressing Specific Symptoms
  • Nausea: Try ginger tea, ginger candy, or peppermint tea.
  • Bloating and Gas: Use over-the-counter aids like simethicone (Gas-X) or alpha-galactosidase (Beano).
  • Constipation: Ensure adequate water intake, increase fiber, and consider using fiber supplements or stool softeners under medical guidance. [1, 2, 3, 4]

Medical Management
  • Lower the Dose: If symptoms are persistent, ask your doctor to reduce the dosage or slow the rate of escalation to allow your body time to adjust.
  • Timing of Meds: If taking oral semaglutide, ensure a 30-minute window before eating or taking other medications.
  • Review Medication: Medications like Zofran can be prescribed to manage severe, short-term nausea

I already went through years of this after the sleeve surgery...now thousands of dollars later, I have to embrace nausea again? And millions still believe in karma. Just-world hypothesis is a fallacy, and karma only exists in strip joints.

If you have not already done it look up https://glp1plotter.com/. It gives you a graph of simulated drug levels over time. Because the way these drugs work and their timing is totally different to any other drug, it is not intuitively obvious, and seeing it on a graph makes it easier to understand.
But it is normal for drug levels to build up over 4 weeks since the last dose increase, so getting worse side effects a few weeks after a dose increase or starting is not unusual, I would expect it at 4 weeks rather than 5 but everyones' reactions are different

Not to beat it to death, but I have different kinds of nausea.
One is the "over-full" feeling. (GLP1 related)
The other is like motion sickness. 🤪 If you've ever had food poisoning, you know.
I've done long north sea passages, where I was fine for days, and then one thing will trigger it. Could be completely not GLP1 related. Only you're gut knows for sure.🙏
Don't dehydrate.

Oh hey Sasquatch, thank you for sharing your valuable insights, but this particular detail was all I could focus on. As a fellow cryptid, I am immediately intrigued.

I crossed the Labrador to Reykjavik, earlier this month.
I'll be over in the Bering Sea, end of summer.

Was just thinking about you and this post and wondered how you're getting on now?

I'm in week 6 (4mg now) and having similar issues 🥲

Just do a reset
skip your next dose till you can eat normaly
and restart your dosing at 1mg as soon your craving become too noisy
and do another 1.5mg 2 days after your first pin of 1mg to asses your sides
when this is well supported back to normal schedule
