Many times those are “family sized” vials for folk w/2+ people in the house on peps. I’ve used a pen for 18 weeks because it held 18 doses; use good aseptic technique and wash your hands plus keep your reconned peps in the fridge and you’re good.

I use 8 or 10mm, 29 g for spicy and larger volume peps (NAD+, Glutathione). I use 5 or 6 mm, 30-32g for “regular” peps. When I started I used 10-12mm needles for all, but I’m fluffier than many (at least the injection sites are). Thinner folk will likely find shorter needles more comfortable.

I could see that happening if you were using large amounts, but a half a cc in 3 mls isn’t going to affect you systemically. If it did, lidocaine patches wouldn’t be OTC.
