I only take the top off when I'm reconstituting for a nasal spray, even though I get gray peps, because I'm a cheap bastard.

IMO it’s okay to take the top off if you’re carefully pulling out the last dose to use pretty much right away. This is no different than using an ampoule. (Cover your breathing holes!). I don’t like to waste any of it, not just because it’s money, but sometimes it means not needing to open the next vial yet. The insulin syringes are much better than luer locks for minimal waste lost to the syringe and needle hub as well.

I "loaned" a girlfriend a 22mg vial of compound (not grey) tirz to try while she decides whether or not to get on it. (if she does, she can pay me back a vial, and if she doesn't, no big deal, I have so much grey in my freezer.) I've done it before for girlfriends who are hesitant to pull the trigger and just want to see if it agrees with them, and they always wind up getting their own and giving me one back,

I talked with her, we pulled the risks/side effects page from one of the pharmacies, I made sure she was aware of all of it, and I showed her the titration schedule. Made sure she knew that it was 10 units for 2.2 mg to start (I know 2.5 is fine, but it's easier to remember the 10 units, and that's what Southend has on their sheet.) Let her know she could text me if there was anything she didn't remember and/or couldn't find.

She went home, forgot the units, dosed WAY too high, and was ill.

I know her to be smart, capable, thoughtful, and competent. And holy shit am I glad it wasn't worse than it was.

Dont pull top off. Flip vial upside down (while needle is in) and pull your needle out so only the tip is still in the vial. Like its almost flush while drawing out the peptide. This will get 99+% out.

To add to this helpful post, there’s typically a narrow slit on opposite sides of the rubber stopper that will help you see the needle tip.

I couldn’t find quite the right emoji to respond to that story. This kind of mistake happens very very easily to people, despite it being kind of mind boggling when you KNOW you’ve been so careful to go over everything. Professionals giving detailed instructions encounter this issue as well. Only in the medical field do people get trained on the spot then sent home. My car mechanic does not send me you home with a wrench.

If this just happened, you can help the situation turn out emotionally better. (And decrease the tendency for her to defensively shift blame onto you). Check in on her frequently. Get her some electrolytes and tell her she can get Zofran from an urgent care. Remind her she needs to eat a little to avoid gallstones. Tell her to get medical help from an ER if she’s getting dehydrated or experiencing excessive pain. Express lots of empathy.

Plot this out on shotsy or the Peppy’s half life plotter for her and help her take advantage of the few weeks of the proper effects while this dose is wearing off. And hopefully she’ll file this away as an avoidable side effect instead of it putting her off the class of meds.

In the future, you might have to actually help her with doses until you both have confidence she knows her level of competency.

I don't feel responsible or guilty, only VERY aware that it could have been much more serious, and with a new appreciation and respect for my usual personal rule that I'll hand someone links to the information where they can get themselves some, but NEVER will hand them product. The fact that it wasn't grey seemed made it seem fine, but ope, now compound counts in that rule too.

I only even mentioned it as an underscore to "man, people do wild shit in lieu of reading and following the directions."

I’ve had to do that with vial stoppers like this to get all the liquid out of the vial. I use a syringe filter then inject into a pen cartridge.

Would a 1.5 inch needle on your luer lock syringe that reaches all the way to the bottom of the vial make your life any easier? I would HATE trying to play "match the needle with the very bottom of the stopper to get all of it."

It’s a 10ml vial and I can’t get it all out with the 1.5” as the stopper height limits the needle angle and I can’t get to the bottom edge of the vial. It’s on my NAD+ vials only.

oh, well hell. That's not what I want to hear; I got some 10ml vials that I haven't opened so I can use them to mix up my KLOW next time, and I didn't even think to see if they're taller than 1.5 ml, OR if the stopper is thick! Sound like I'm likely to have 10ml vials to store glass beads in instead. 😀

Oh, I didn’t mean for my response to make you less likely to help people find out about glps, only to kind of mention this is just what happens sometimes because people make mistakes. And if you can avoid the trap (sometimes legal) of feeling inappropriately responsible for other people’s actions, there are just helpful ways to follow through when these things happen, when we cared enough to start talking about this path. (and not that doctor or parent is a role limited to doctor or parent, sometimes a friend needs some help.)

I think everyone makes odd mistakes even with the best of intentions, so I’m not blaming her either. I’m assuming that the evaluation the two of you made together prior to her starting was that it was overall a beneficial choice, and so despite a rocky self inflicted road, it was a good decision.
