I've been on 15mg tirz for a long time now, probably about a year. Effects aren't as strong as they were and I haven't been able to lose and am even struggling to maintain. I initially lost 85 lbs so it definitely helped, but im wondering what I can do next.

I worry about adding Reta because of chronic kidney disease, which I read is a reason to avoid reta.

My food noise is terrible. I miss not thinking about food all the time. Is it better to just increase the tirz dose? How high? Are there other options other than reta? Would sema be worth stacking?

Thanks!

Some folks on here have gone above the 15mg limit set by EL. And there are studies being run on higher doses.

These are for sure some hard questions to have a definitive answer. Not everyone reacts the same.

One strategy is to back off your dose a bit and pin every 4 or 5 days. I do this because I was having suppression until day four and was struggling on days 5 and 6. Before trying this, when I increased my dose to 15mg, I started to get sides. When I added Survo at a low dose, I got sides.

How are your sides? A trialed max dose could be higher than an individual’s tolerable max dose… i suspect the opposite is also true.

I’m at 12mg weekly of Reta, no adverse sides. I’m taking grey market non approved peptide… nothing I’ve seen or experienced suggests a reason to not escalate dose as tolerated. I’m not beholden to the trials. I have no apprehension of going beyond 12mg/week.

If tirz has been working comfortably… seems easy to increase dose unless you already have other options at hand.

Have you tried sema? It helps a lot with food noise. I'm liking mixing sema an tirz.

I occasionally add a smidge of Sema to my Tirz dose, and it helps with the food noise and appetite.

That said, the clinical trials indicate that weight loss stops/tapers off around 72 weeks, so you may have just reached your max loss.

Thats a good point. My main sides are fatigue and sometimes headaches. Hasn't occurred in awhile. It was usually when I increased dose

Sounds promising. I assume you have an ample supply of Tirz? Do you have Cagri? That would hit a different receptor vs sema being more GLP.

Since my third week on GLP, I’ve split dosed 2x weekly (the past 11 weeks). I’m cruising at an average 2.8lb/week loss, currently at 242lb. I have plenty of cagri, sema, tirz, and reta. The plan for my next step is advancing to 14mg/week.

A theory of a plan after I get to comfortably to my target weight/condition is to either titrate down, or a more interesting idea is a ~2-3 month sabbatical/reset and restart at a lower dose. I’m listening to others reports of their reset, I’ve got plenty of time to develop my plan 😂

I have a decent stock of tirz but that's all, I was looking into what else to add or switch to before I purchased anything else. I see a lot of people suggesting adding sema but im not sure if that would be as effective as cagri or just switching to semi completely.

I've been wondering about trying that. How are you doing it?

I've been taking about 17mg of tirz for awhile with no adverse effects and actually started stacking with sema back at the end of November. I was using a small dose of sema, maybe .5mg or 1mg and it was going really well and then my sleep got messed up. I blamed it on the sema, so the last couple of weeks I skipped the sema. My sleep is still messed up, so I'm now thinking that it wasn't the sema 🤣.

Since you've been on 15mg of tirz for a long time, I think a stack of sema would probably be better than more tirzepatide. I didn't notice much difference between 15mg and 17mg. My plan was to go up to 20mg, but then I did the sema stack and think that that was a better plan.
