I have been taking tirzepatide for a little over three months and have ran into problems that are stopping me from getting to an effective dose. I worked my way up to a dose of 5 mg of tirz once weekly. About three days into the week, I got debilitating diarrhea and bothersome insomnia.
When this occurred I thought the gastro-intestinal problem was due to an incident eating out the day before. However, in three days or so after taking my next tirz dose, I again suffered the gastro-intestinal problem. The insomnia may be anxiety related and there is a fair chance over time I could resolve it. The other problem is just to difficult to cope with. Therefore, I have dropped down to a 2.5 mg. tirz dose split into two halves pinned twice weekly. Geting some reduction in food noise but not enough to be effective. I am considering switching to reta because the effective dose is so much lower than it is for tirz. I am hoping the desired beneficial would kick in at a lower level than the nasty sid effects. Love to hear from you reta users. Is there ant chance this strategy would work or is it just a pipe dream? I would appreciate any thoughts, comments, or suggestion that you have. Thanks in advance!

I'd say it's worth a shot🙂 maybe start with a low dose kit or single vial so you're not too invested if it doesn't work out.

Yes, go low and slow, side effects do mostly pass. I remember the tirz GI side effects, oh boy.
And it passed for me.
But not for everybody , but do give tirz a shot. It’s such a good one

I haven't taken Tirz but ran into similar issues way back when on CagriSema. How's your water and fiber intake? Diet in check? And moving your body regularly? Feel like there's a few variables that can contribute to what your going through - before changing meds. Agree, with the above - if all else is in check, maybe try just a vial of Reta before going full kit

Obv it stands to sense this is your choice, and every body is different.

My read of your goals are:
  • less or no GI side effects
  • less or no insomnia
  • suppression
  • food noise reduction
  • low dose as possible

The food noise suppression at the low doses of Reta may not work for you, and having been on tirz, you may find you need to take more Reta (beyond 2-5mg) to get any effect at all. GI side effects may be more or may be less that one you won’t know about unless you try. I would recommend split dosing to assist with this ( as you are with tirz) and starting low on Reta to see how you tolerate it. However doing so will mean you may well get a return of food noise and the suppression will likely be less and feel different.

I think the experience/ feelz consensus from other glp users here which have tried both ( including me) appears to be Reta does raise a lot of people’s resting heart rate and some complain with insomnia as a side effect. That’s people who didn’t get it before on tirz.

What I would say is if you do try Reta, give it time and patience to work, for best researching experience you should let the tirz completely leave your system, before switching, but few have the patience for this, so many don’t. This is one line of the “ official” advice from prescribers of glp1’s for people who switch. There is obviously no guidance for Reta switchers. The tirz takes time to leave your system fully too so be mindful of that initial overlap on your GI symptoms.

Everybody is different and Reta may work beautifully for you but it’s good to be realistic and weigh it up pros and cons quite carefully. Somethings you can only know when you know them and you try it out.

The effective dose argument is different. It has to work for you to give you the desired outcome.

Yes the studies in test conditions say that 15mg of tirz is most effective yielding biggest loss in trial participants, I titrated up with an official prescriber at their recommendation. But lost most weight on 5mg and should have stayed there longer.
Study upper dose was 12mg for Reta, but effective between 4-8mg when balancing out side effects.

but 15mg of tirz was a massive plateau for me. I hardly lost anything. For more that 5 months nothing. Reta wasn’t working for me at all ( re: weight loss) until I went to a single dose of 6mg. Which is now working well 🙏

I too hoped I’d be lucky and could stay at 2-3mg Reta for a while when making the switch, so I get your thinking.

Didn’t work out like that for me. But it may work a dream for you! some people seem to super respond to Reta ( 0.25 and 0.5ml giving good effects) but it seems to me that their journey has been that they haven’t tried other GLP1s ( could be wrong there will be exceptions ).

Wishing you well for whatever choice you make and that you find the right combo that hits the mark for you 🎯
