I've realized that there are many posts about this topic. I'm feeling a little bit down and would like some opinions. I've been on tirz for7 months and lost 68 pounds. The past 4 weeks I have been at a standstill. I've been taking 11mg tirz , up from 10 mg for 2 weeks now. My first week at 11mg, I dropped 3 pounds. My second week, 3 days in, I've gained a pound and a half. I have about 40 pounds more to lose and becoming quite frustrated. I would like to add some sema, since I have it already. I drink the water, get 60 to 100mg protein in a day whether it be from actual foods or shakes.....I'm not very active. I am 53 years old and have heart issues. I feel by adding sema to the tirz is fairly safe for me. I love this forum and learned/am learning a lot. I hope that you can give me some pointers. What do you think? Thank you!

I am curious about this too. I had noticed on a strict calorie deficit I would not lose weight even after tracking for several weeks, I would try to have a "cheat" day to fool my body into thinking I was not starving, I cannot say if that is what worked. I also tried to go longer than advised between titrations and also smaller increases. I have no idea what worked, but it has worked and I have had a very similar experience as you. 57 with 61lbs down currently at 10mg contemplating 11..lol.

One of these seemed to work because I always popped out of plateau..

I have seen people post about EL strategy of titration being for a reason, both in dosage and timing. Maybe it works better if you do not let your body get used to it too slow? Obviously everyone seems to have different results, and some never go above 5mg while others don't get real results until 12.5mg.

I would love to hear more feedback, anecdotal or research based.

I've always heard ppl saying that they add sema when food noise returns but I would be curious to know if it helps break the stall as well. I moved up from 10mg to 12mg Tirz about 3 months ago. I only have 13 lbs left to lose but I am struggling with each one. I am lucky to lose 1-2 a month and I barely eat as it is. Please keep us informed if this is what you choose to do. Good luck!

Maybe that's what it is? I don't eat that much, because I can't. However, a day or 2 before my next shot, I can probably eat a little hardy. Maybe my body is trying to hold onto the weight due to the calorie deficit? You've got me thinking now. It really is something to think about. When I do finally increase my tirz dose, I'm skipping 12 and going right for 12.5mg. I really appreciate you sharing your experience. Good luck to you!

I do think that the closer that you get to your goal, it becomes harder to lose the weight. It's also very frustrating, non the less. It's hard for me to eat for the first 3 or 4 days, too. I will let you know how things go. I thank you for your well wishes. Good luck to you too!

When my 5mg Tirz was feeling light day 5/6 I started Cagri. I didn't want to overlap or duplicate. Now that I'm at goal I'm reducing Tirz and adding Sema honestly just to use up what's in my freezer... I've got nothing to lose! 🤣

Now I can tell what adding Sema to Tirz might have resulted in (dosing weekly same day). Increased suppression the first few days yet still the day 5/6 decrescendo. I'll eventually take just Sema and Cagri until it's gone then return to Tirz (my favorite) or I'll gift it to someone who's been cut off compounding. My take on Sema so far is it's more rustic, not as smooth as Tirz (but I like it). Glucose control not as steady but still great! YMMV

Thank you for letting me know that. I thought that this would be the safest route if I were going to add anything. I'm happy that you let me know how and when to use it and that you really like it. Thanks again!

Hello again 👋 Just so you know, I just pinned .25mg of sema. I got on the scale today and I am a pound more than yesterday. I know that it could be water weight, but I already pinned Tirz this past Friday. I don't know what to do? I will keep you posted. Thanks again!

Im interested in your tirz + sema stack! I'm about to do the same.
Increasing to 10mg of tirz and adding 0.25mg of sema 4 days later.
I was having great results with tirz but for the past few weeks Ive been ravenous, food noise, no satiety at all. I could increase tirz even more but want to see if sema gets the edge off for now.

For context, 47F 5'1.5 CW 133 SW 184 GW 110. Roughly 30% weight lost so far, 8 months total.

I do wish you luck. Looks like you're doing fabulous 👍 I think sema will definitely help you with your appetite and food noise. I'm not eating very much. I just don't know what to do anymore. I'm going to try this stack first. I was doing great. Now, not so well. Maybe my body is desensitized to tirz. I'm just trying to get a good Kickstart. Good luck and please keep me posted. I will do the same. Thanks 👍

Maybe you are experiencing a plateau, it’s a normal part of weight loss. I doubt your Tirzepatide has quit working, a lot of folks experience this on name brand too. I had a period like this too, but just when I thought it was time to move up doses I started losing again. Sometimes our bodies just need time to adjust to all of the changes that they are experiencing. Just my humble little opinion.

That could be it too. I just get so afraid that I will end up where I was before. It then becomes frustrating. I need to have more patience. Thank you

I get that 100%! I'm at maintenance and have started stacking Cagri with my Sema for food noise because I had gained a few pounds. If things don't start moving for you soon try the stack, but one thing I would suggest if you can do it is to log your food. It helps you be able to see what foods help you lose better, for me its protein! Best wishes.

That's a great idea! I think that I just might do that. Thanks for the idea and good luck with your maintenance thumb

I stack sema with my tirz and it has been a great combo. Planning to start adding in a dash of cagri for these last few stubborn pounds.
Do you mind me asking, what's your stacking regimen right now? Do you take tirz and sema at the same time?

I've been thinking a lot lately about why I'm struggling so much right here at the finish line. I know the Tirz is working because I am NEVER hungry. My stomach has not growled in at least 8 months or more. No joke. I use the Samsung heath app and wear a galaxy 7 watch to track all my food, exercise, sleep, etc. Most days my calories are 800-900. I know that's low but I have always had to eat less to lose even when eating healthy. I make sure to get my fiber, protein, good fats and stuff I need plus I take supplements just to make sure. I know most of us suffer with constipation due to the meds slowing everything down. I'm starting to belive that's a big part of the problem. A few months ago I started taking cherry Dulcolax every 2-3 days because I felt so miserable. I notice that I always lose when I weigh myself the day after I take it. Makes sense. After going through constipation on/off for months or even years how much is stuck to your intestines and bowels that just doesn't come out? I've read different reports that's some people who suffer from chronic constipation or IBS-C can have as much as 15-25 inside of them. I feel like this might be the answer I've been searching for. I could be wrong. God knows it woukdnt be the first time. I just don't know how to find out for sure without going to one of those clinics where they clean you out. If anyone has any SERIOUS ideas plz lmk.

For my last colonoscopy instead of picking up the prep jug at the pharmacy I was given these instructions:
  • Four Dulcolax® laxative tablets containing 5mg of bisacodyl each (NOT Dulcolax stool softener).
  • One 8.3-ounce bottle of MiraLAX® (238 grams) or generic equivalent.
  • Two 32-ounce bottles of Gatorade®. (NOT RED).
I prefer the jug (adding a packet of Crystal Light lemonade). By transitioning to OTC I had to pay out of pocket which is not a big deal but the jug is definitely a more definitive cleanse. If I ever had a problem like some here I'd resort to the above.

Thank you for replying seriously. I think I am going to give your suggestion a try. I would much rather do this in the comfort of my own bathroom instead of having to a stranger stick a tube up my a** and watch my 💩 come out. I so was not looking forward to having to go that route esp after I researched it. 😫

I like your idea too. Every day I take 5 fiber gummies and drink a 16.9oz bottle of Metamucil. I also drink 1 or 2 protein shakes with fiber. I do try to eat something natural with fiber every day too. Some days, I don't eat that much. That is very good to know. It would be good to do from time to time.

The only benefit of the Gatorade route is less liquid (64oz). If I recall the pharma jug "Go Lightly" (oxymoron) is a gallon.

They may have taken the OTC route with me because I have SIADH and metabolize liquids as body mass. If I don't restrict fluids my sodium level becomes critical.

That makes so much sense! I think it's so good to know for Spring cleanup 😄

This is one of the most important topics taking these meds all around. I'm always inking Metamucil every day and eating fiber gummies with fiber protein shakes, of course the water, too. I love the regimen nonyabizznez gave. That should move things along. You'll probably drop a bit of weight and feel a lot better.

For me, prevention is the way to go. I take about 1/2 to 3/4 of a dose of Miralax almost every night. If I don't the consequences are brutal. My doctor has okayed it, I've had to use it since starting in 2023.

Miralax is good too. That's the only thing when people are on these meds. You have the right idea. Prevention is so important. It can't be stressed enough. A lot of new people haven't a clue. I know this, I was new once, too. It can go bad fast.

I've been stacking sema and tirz for a couple of years. I also take metformin a few days a week. I've lost 150lbs. I don't think I had any huge stalls during that time, but I'm in maintenance now and not looking to lose anymore. I still take both because I'm worried changing anything up might result in weight gain. I'm one of those people that will need to be on some glp-1 for the rest of my life because I was fat my entire life. . Just if it's not broken don't fix it type of situation. I take 15mg tirz every 10 days and then 5 days later take 0.5mg sema (although I might drop down to 0.25mg since not sure if I need to stretch out supply).

Congrats on the weight loss and maintenance! 👏

So you're taking .5mg Sema once every 20 days? (And basically 10.5mg weekly Tirz). Also is the Metformin still for glucose control or did the GLPs take care of that? (Or maybe you don't know if you never went off Metformin in your process?)

You've been taking 15mg tirz every 10 days and 0.5mg sema after 15 days? Have you always stacked tjis way or do you take this dosing combo for maintenance? I'm just curious. Thank you for sharing!
