New to this miracle of miracles. When I was sitting at my desk at work, 20 feet from the staff lounge, where there was a table of cakes, cookies, and muffins some bastard brought in and abandoned (WTF?) but, yet, against all odds, I didn't even think about sneaking away from my desk to load up a plate of refined carbs, I knew I had entered into a strange new world. I've started to go down the rabbit hole of sources from compounding, to resellers, to going grey. As well as discovering the whole candy store of fun, multi-flavored peptides that can be stacked on the counter, but are down the line when I'm done shedding 70 pounds and have saved more pennies. What what a long, strange trip I'm on. Not to mix metaphors of ups and downs, but I'm also climbing the staircase of GLP's and their ilk. After a very short stint of sema, I'm on tirz for now but have retta on the way when the tirz runs out, but it can hang out with my frozen peas and carrots for a while, and now I'm keeping my eye on cagri to see if it might be a friend to tirz or reta when the going gets tough and the pounds over stay their welcome. Thanks for having me and I appreciate everyone's humor and advice. I'm going to need it.

You are amazing. In my opinion, controlling diet is a very difficult thing. We have a lot of delicious food here. I can't help but want to eat a lot of food every day.

Love it bro! Keep up the good work. I too am looking at switching to Reta once my Tirz goes dark…
I just started Reta. I feel like I’m the only person in the world that didn’t lose weight on Tirz. I was on it for 6 months and lost zero pounds. Minimal appetite suppression. Hopefully Reta is as amazing as everyone is saying it is.

Did you try up to 15 mg of tirz? Did at least your lab results improve?

Some labs that may help:

Here are some labs that can help explain why someone might not be losing weight on tirzepatide:

1. Thyroid Panel:

  • TSH (Thyroid-Stimulating Hormone): This is often the first test for thyroid function.2 An underactive thyroid (hypothyroidism) can slow metabolism and make weight loss difficult.3

  • Free T3 and Free T4: These directly measure the active thyroid hormones in your blood. If TSH is abnormal, these provide more detailed information about thyroid hormone production and conversion.
  • Reverse T3: Can indicate issues with thyroid hormone conversion, even if TSH, T3, and T4 appear normal.4

  • Thyroid Antibodies: To check for autoimmune thyroid conditions like Hashimoto's, which can cause hypothyroidism.5
2. Insulin Resistance Labs:

  • Fasting Glucose:Measures your blood sugar level after an overnight fast.6

  • HbA1c (Hemoglobin A1c): Provides an average of your blood sugar levels over the past 2-3 months.7

  • Fasting Insulin: This is crucial for detecting insulin resistance, even before fasting glucose or HbA1c become abnormal.8 High fasting insulin suggests your body is producing a lot of insulin to try and control blood sugar, which can lead to fat storage.9

  • HOMA-IR (Homeostatic Model Assessment of Insulin Resistance):A calculation using fasting glucose and fasting insulin to assess insulin resistance.10
3. Hormonal Imbalances (beyond thyroid and insulin):

  • Cortisol: A stress hormone.11 Chronically elevated cortisol can lead to increased fat storage, especially around the abdomen.12 A morning cortisol level is a good starting point.

  • Sex Hormones (Estradiol, Progesterone, Testosterone, DHEA-S):Imbalances in these hormones, particularly in women (e.g., Polycystic Ovary Syndrome - PCOS), can significantly impact weight. PCOS, for example, is associated with insulin resistance and difficulty losing weight.13
4. Nutritional Deficiencies:

  • Vitamin D (25-hydroxy Vitamin D):Vitamin D plays a role in metabolism, thyroid function, and blood sugar regulation.14 Deficiency is common in individuals with obesity and can hinder weight loss.

  • Complete Blood Count (CBC): Can check for anemia, which can cause fatigue and impact energy levels for physical activity.15

  • Iron Studies: If anemia is suspected.
5. Inflammation Markers:

  • hs-CRP (high-sensitivity C-reactive protein): Chronic inflammation can contribute to weight loss resistance and various health issues.
6. Liver and Kidney Function:

  • Comprehensive Metabolic Panel (CMP): This panel assesses liver and kidney function, electrolytes, and blood sugar.16 Issues with these organs can impact metabolism and overall health, potentially affecting weight.
7. Lipid Panel:

  • Total Cholesterol, LDL, HDL, Triglycerides: While primarily related to cardiovascular health, abnormal lipid levels can sometimes indicate underlying metabolic issues, including those related to thyroid or blood sugar management.17
8. Gut Health:

  • While not a standard "blood test" for weight loss directly, a GI Map Stool Test or similar functional gut health tests can assess the gut microbiome.18 Imbalances in gut bacteria can influence metabolism and inflammation.19
Important Considerations:

  • Dosage and Time: Ensure you're on an appropriate dose of tirzepatide and have given it sufficient time to work (typically 6-8 weeks to see initial results, with significant loss over 72 weeks).
  • Lifestyle Adherence: Reinforce consistent adherence to diet (focus on whole, unprocessed foods, adequate protein and fiber), regular exercise, sufficient sleep (7-9 hours), and stress management.20

  • Medications:Some other medications can contribute to weight gain or make weight loss more challenging.21 Review all current medications with your doctor.

  • Genetic Factors: While not a lab test in the traditional sense, genetics can play a role in how individuals respond to weight loss interventions.
If you are not losing weight on tirzepatide as expected, it's essential to have an open conversation with your healthcare provider. They can evaluate your overall health, review your lifestyle, order appropriate lab tests, and adjust your treatment plan as needed.

I think I went up to the second highest dose, maybe? And I don’t recall if I had lab work done while I was on Tirz, but my labs definitely improved on Sema. I was in Sema for 6 months and lost 25 pounds but I switched to Tirz because everyone said it was better. After six months with no weight loss my provider and I felt it was best to switch me back to Sema since I was getting results on that. Then I lost an additional 20 pounds on Sema but stalled about 6 months ago. So now I’m trying Reta as I still have weight to lose.
