I started Triz on Oct. 11th at a 1mg dose to see how I would respond as far as side effects. I didn't have any so I went to 2mg this last Saturday which I know is still below the regular starting dose. Seems like this week my hunger has increased again. I just ate a tuna sandwich and I feel like I need some more. But to be honest I really have not had much to eat today and that may be why. But my calories totalled about 1300+. So should I go up next week or maybe add another .5 mg now? Need some advice! Thanks!!

Not questioning your decisions but why not start at the regular 2.5mg dose? I know you said you wanted to see how things went, but even 2.5 isn't considered a therapeutic dose. I'd go to 2.5 and titrate up to 5mg after 4 weeks.

thanks. I think I let the fear of the side effects make me want to start low. The last couple days have been the worst for hunger. I will do the 2.5 this Saturday or maybe a little sooner if this hunger keeps taking over.

The recommended titration schedule from Eli Lilly is 2.5mg weekly for four weeks and upping the dose 2.5mg every four weeks. So, you've been under the recommended protocol since you started. They are trying to make bank off that molecule, so they've put a lot of money into figuring out how best to make it work.

In the first month (even at 2.5mg) it's common to start feeling hungry later in the week before your next dose. This is because the weekly dosing schedule eventually works with the 5-day half-life to reach a plateau of drug concentration after repeated dosing. That plateau or "steady state" doesn't arrive for 4 or so weeks (hence the 4-week schedule for bumping the dose). You're only on your third dose still going up and down, and with so small a dosage, you're going very far down.

Also, peak concentration after a dose can be anywhere from 8 to 72 hours depending on your body. It's not something that you can "pin 0.5mg because I feel hungry NOW" kind of situation. It's not immediate.

Don't try to out-geek the Big Pharma geeks with a DIY titration schedule. I'd pin 0.5-1mg today, consider last Saturday your first dose of your four weeks at 2.5mg. For week 5, assess if you want to bump dose to 5mg. You will be approaching a steady state of concentration and will not be seeing a fluctuation in your appetite because of it. Then do 4 weeks at 5mg to arrive at the steady state for 5mg and assess then if you want to continue or if it's a good level for you.

Some people don't see side effects until higher doses. But for tirz, go with the regular doses. If you get side effects on 5mg, you can titrate down to 2.5 again. Then stay on it a few more weeks and go back up to 5. If you don't notice any side effects on 5, go to 7.5 in 4 weeks and so forth.

Some people will say to stay on your dose until you stop losing...I'm just following guidelines from EL.

I always feel bad for people who have issues with hunger. A day or two after taking my first 1.5 mg dose, I could definitely tell it was working. Have since switched to Reta and have never once thought between the two they were not working.

Are you buying grey? If so, are you getting it from a reliable source with COAs and/or third-party testing? When I hear that people are not responding, I always seem to want to question the source first.

That was more elaborate and detailed of what I was saying 😅, so thank you for that.

That was great advice Sparky

Agree, go up you are too low.
🙂
You hear a lot about side effects, but all the people who don't have any or minor side effects, don't post about it.
