Mid-afternoon slump

I'm just into week 4 on Tirz and am having a surprisingly fast and effective reaction even on this starting dose. I've lost a couple of kilos effortlessly. One downside I have noticed that as mid afternoon approaches I am getting incredibly tired and getting headaches.

I'm tracking my food and I'm getting a solid amount of protein and fat. I think therefore that the main issue is a lack of carbs, but I can't really face eating too much more - the food noise is not the same as it was and my enjoyment of it has decreased too (grateful for this as it was ruling my life in an unhealthy way before). I tried to add some extra carbs into lunch today but I still feel foggy, groggy and tired so it clearly wasn't enough.

Does anyone have any tips for how I might integrate more carbs into my diet without it feeling like an absolute slog?
 
I had headaches when I first started tirzepatide and struggled for a few weeks with nothing really helping. Eventually I tried tramadol, and that cleared them up completely — never came back after that. So if you have access to something a little stronger than OTC options, it might be worth trying. No fatigue issues for me on tirz, but I do notice it on cagri, so that tracks with what you're experiencing.

On the carb front, I was firmly anti-carb for a long time. What shifted things for me was working with a nutritionist who helped me reintroduce them gradually. Now I do fine with some carbs as long as I focus on complex ones. For example, today I'm having chicken tenders with corn and coleslaw. I'll use honey mustard or sweet chili sauce, both of which add a little, but it doesn't derail my progress.

Tramadol is always fun so who am I to pass up that recommendation! I think I do have some lying around from a previous back issue so I'll have a dig around.

Corn was what I integrated into my salad at lunch today, I probably could have been more generous tbh. Olive oil is my usual go to dressing (see country flag!) but yeah, maybe I should branch out and add some more fun in. Thanks!

Yes, I saw your country flag (my favorite place in Europe) and was going to make some comment about carbs, because whenever I'm in Italy I feel like I eat too many carbs, so I was surprised that you were having trouble finding carbs.

I was just in Rome/Florence earlier this month for about 10 days. And have two trips next summer. This trip I can't remember a single piece of protein that I ate 🤣. I can't eat Bistecca alla Fiorentina because it's too raw for me. And in Rome, I'm addicted to pizza al taglio, suppli and gelato and could live off that without any meat consumption.
 
Italian carbs - the reason I turned to Tirz! For now the love affair has come to a halt but hopefully we'll be reuinted soon.

I personally found that my first month or so on Tirz actually helped with my early afternoon lull - probably because it helped me stop overeating at lunch and avoid the resulting cycle of blood sugar peaks and valleys. But then as I got further into being in a sustained weight cut, some of that afternoon drowsiness started coming back. Honestly I haven't found a great solution to it yet, other than extra caffeine around lunch time (and lack of carbs is not my issue, lol). Have placed an order for Semax / Selank and will be giving that a go shortly.

IME simple carbs are the easiest way to fix (if you're certain it's a carb issue..). Have a nice cookie or piece of cake (if you're not too concerned about the quality of the carbs) or carby sweet of your choice.

Surely those are not a chore to eat even on a GLP! 😀

Prior to that make sure your diet and micro nutrients are dialed in.

I have looked into this too and had it on the list, though was leaning towards one of the precursors, likely NR.

Precursors are certainly easier and work well for many. Never did anything for me however NAD+ has definitely helped. But yes, I agree and would start by trying the precursors if going the NAD boosting route.

Do you think an OTC supplement is enough for the NAD+? I'm not keen on adding any additional pins straight away while I'm still feeling out the Tirz but I would like to feel less like crap ASAP!
 
Third week, raised to 3.5mg and feeling the same, lets see if NAD+ would help, it's coming. BTW carbs def help.
 
Update, third NAD+ dose, 50mg each. Def helping during day, but once you try to do any kind of sport ahaha ciao ciao. Feel like a shit in 0.2
