Hi all,

I'm currently on a 6-day rotation for Reta and it has been working great. My spouse is taking 2.5mg of Tirz on a 7-day schedule and towards the end she starts to get hungry and I am wondering if it makes sense to go to a 6-day rotation? Right now, she's losing weight slowly and is OK with it so she's hesitant to bump up to 5mg and just wants to avoid the food noise at the end of the rotation.

Thanks

6 days is fine, some people even do 5. Or split the dose every 3.5 days. I do that and im never really hungry

Doc explained it thusly. It takes a few days for tirz to achieve maximum function for the dose. Then at five days it's half as effective as at maximum. The unknown factor is how long it takes to ramp up. But with experimentation they designed the dose for weekly for most. (not all, obviously) Just like they designed the dosage to be 2.5 to 15mg in the same .5ml dilution.
I try to last a week, but I prefer to give myself a bump day 4 or 5, with 3 or 4mg dose. Mind you, I'm at 12.5mg.
Because I had a lengthy stall I asked about a 5 day instead of 7 day regimen. My concern was a "loading" factor to which he explained away. (see above)

The weekly schedule devised by Lilly is surely more about patient compliance and ease of use than drug half-life.

I personally think 5 days would be great, but it would be difficult for me. I travel for work a bit and away from home several days most weeks. And it would be a pain to bring the medication with me. I also like having maximum coverage over the weekend when I’m most likely to cheat on my diet. I also have trouble remembering to take my meds in general, despite reminders and calendar entries. A once a month shot would be fantastic. That I could handle.

Personal experience, initially, 2.5mg I could feel immediately. Probably part placebo effect. Later, it felt like a curve, where there's a peak at 3 days only lasting a couple days. I was always given the ability to titrate as I felt. It's not been a straight line. I had a lengthy stall. Seems I'm pretty resistant. So, consequently, micro-doseing never was thought of as an option. Now, that's not to say for anyone else. Everyone has to go by feel. Should I say there's a difference between how your body feels and emotions? That's tuff, bc we know the gut is directly connected to your brain. Merry Christmas 🎅 🎄
